              EXTN-QF-08-                      1. Training or seminar proposal will be prepared by the College,

                                               3. Upon approval, the college, Campus, Unit or Training and
                                                  Courseware Development Division of Extension Services will
                                                  implement or conduct the seminar or training.
                                               4. The Extension Services Monitoring and Evaluation Division
                                                  facilitates the evaluation of the training using training
                                                  evaluation forms (EXTN-QF-09, EXTN-QF-10) to assess the
                                                  strengths and weaknesses of the training program including
                                                  the course content, resource speakers, management and
                                                  venue based on the perception of the participants.
                                               5. Once the training completed, the Extension Services- Training
                                                  and Courseware Development Division prepare training
                                                  proceedings using the prescribed format (EXTN-QF-11).

                          All  research  and  extension  projects  shall  be  properly  endorsed  by  the

                   concerned office to the Campus Research Development and Extension Services

                   office  for  recommendation  to  the  Campus  Administrator.  A  Memorandum  of


                   Agreement or Memorandum of Understandings (MOA/MOU) shall be executed for

                   projects involving external partners.
