EXTN-QF-08- 1. Training or seminar proposal will be prepared by the College,
Training_Seminar-Proposal- Campus, Unit or Training and Courseware Development
Format Division of Extension Services using the prescribed format
EXTN-QF-09-Training- (EXTN-QF-08). The proposal will be submitted to the
Evaluation-Form-Livelihood- Extension Services and Endorsed by the Office of the Vice
Training President for Research and Extension to the University
EXTN-QF-10-Training- President for approval.
Evaluation-Form-Technical- 2. Before the conduct of seminar or training, all preparations and
Training arrangements must be finalized. Reservations for the use of
EXTN-QF-11- seminar or training rooms and other facilities should be
Training_Seminar- confirmed. Resource persons/lecturers and participants
Proceedings-Format should be informed on the definite schedule/s of the seminar
or training program.
3. Upon approval, the college, Campus, Unit or Training and
Courseware Development Division of Extension Services will
implement or conduct the seminar or training.
4. The Extension Services Monitoring and Evaluation Division
facilitates the evaluation of the training using training
evaluation forms (EXTN-QF-09, EXTN-QF-10) to assess the
strengths and weaknesses of the training program including
the course content, resource speakers, management and
venue based on the perception of the participants.
5. Once the training completed, the Extension Services- Training
and Courseware Development Division prepare training
proceedings using the prescribed format (EXTN-QF-11).

National Regional, Provincial projects/programs/activities.
and Local Development These forms will be used to align the projects/programs/activities

through UGNAYAN 2.0 Approach”. This will also serve as the
Form 2 Potential Impacts umbrella of different extension projects from colleges and
Form 3 Strategic Plan campuses and will coordinate all the Colleges, Campuses and
Form 4 Plan of Activities Units’ extension activities.
All research and extension projects shall be properly endorsed by the
concerned office to the Campus Research Development and Extension Services
office for recommendation to the Campus Administrator. A Memorandum of
Agreement or Memorandum of Understandings (MOA/MOU) shall be executed for
projects involving external partners.
