SYSTEMATIC AND EFFECTIVE PROCEDURES
Since the second quarter of 2018, CvSU started implementing standard
procedures, also known as quality procedures, as compliance to ISO 9001:2015
standards. Among the highly relevant procedures are the following (Table 2.5):

EXTEN-QF-01 Extension 1. The College, Campus or Unit Extension Coordinator submits
project Proposal Form extension program proposals endorsed by the College Dean,

Report-Format 2. Extension program proposals are endorsed by the Extension
EXTN-QF-03-Annual-Report- Services and OVPRE to the University President for

EXTN-QF-04-Monitoring-and- Evaluation, approval and endorsement to the Board of
Evaluation-Form Regents.
EXTN-QF-05-Periodic- 3. The Administrative Council evaluates the proposal. If it is
Monitoring-Form approved by the Administrative Council, the proposal will be
EXTN-QF-06-Terminal-Report- endorsed to the BOR for approval. If it needs revision, the
Format Proponent College, Campus or Unit will revise the proposal
EXTN-QF-07-Agency-Inhouse- before it will be endorsed to the BOR for approval.
Review-Report-Format 4. The College, Campus or Unit Extension Coordinator submits
extension project or activity proposal based on the approved
Extension Program using the prescribed format (EXTN-QF-
01) and other necessary attachments. All proposals submitted
are endorsed by the College Dean, Campus Dean or Unit
Head.
5. Extension project or activity proposals are endorsed by the
Extension Services and OVPRE to the University President
for approval.
6. The College or Campus implements the project and submits
quarterly and annual reports (EXTN-QF-02, EXTN-QF-03) to
the Extension Services.
7. The Extension Services Monitoring and Evaluation Division
facilitate the monitoring and evaluation of implementation of
extension activities. Aside from presentation during in-house
review, on site evaluation is also conducted. Extension
activities are monitored and evaluated using the prescribed
Monitoring and Evaluation Form (EXTN-QF-04) and Periodic
Monitoring Form (EXTN-QF-05).
8. Once the project is completed, the College or Campus
submits completion/terminal report (EXTN-QF-06) to the
Extension Services.
9. The Extension Services taps experts to review completed
projects. Colleges or Campuses are also asked to present
their extension project during University In-House Review of
Research and Extension Activities using the prescribed
Agency In-House Review Paper Format (EXTN-QF-07).
