Extension Activities using the prescribed Agency In-
House Review Paper Format (EXTN-QF-07).
EXTN-QF-08- 1. Training or seminar proposal will be prepared by
Training_Seminar- the College, Campus, Unit or Training and
Proposal-Format Courseware Development Division of Extension
EXTN-QF-09-Training- Services using the prescribed format (EXTN-QF-
Evaluation-Form- 08). The proposal will be submitted to the Extension
Livelihood-Training Services and Endorsed by the Office of the Vice
EXTN-QF-10-Training- President for Research and Extension to the
Evaluation-Form- University President for approval.
Technical-Training 2. Before the conduct of seminar or training, all
EXTN-QF-11- preparations and arrangements must be finalized.
Training_Seminar- Reservations for the use of seminar or training
Proceedings-Format rooms and other facilities should be confirmed.
Resource persons/lecturers and participants should
be informed on the definite schedule/s of the
seminar or training program.
3. Upon approval, the college, Campus, Unit or
Training and Courseware Development Division of
Extension Services will implement or conduct the
seminar or training.
4. The Extension Services Monitoring and Evaluation
Division facilitates the evaluation of the training
using training evaluation forms (EXTN-QF-09,
EXTN-QF-10) to assess the strengths and
weaknesses of the training program including the
course content, resource speakers, management
and venue based on the perception of the
participants.
5. Once the training completed, the Extension
Services- Training and Courseware Development
Division prepare training proceedings using the
prescribed format (EXTN-QF-11).
Form 1 Complementation These forms are outputs from the workshop for
of Extension Activities for University Internal Impact Assessment of the
National Regional, Extension projects/programs/activities.
Provincial and Local These forms will be used to align the
Development Agenda projects/programs/activities in the newly approved
“CvSU Poverty Alleviation Program through
Form 2 Potential Impacts UGNAYAN 2.0 Approach”. This will also serve as the
Form 3 Strategic Plan umbrella of different extension projects from colleges

Form 4 Plan of Activities and campuses and will coordinate all the Colleges,
Campuses and Units’ extension activities.
